Arrest Information Update Form

Updated process for requesting arrest information updates, takes effect on October 1, 2023
As a reminder, the New York State Division of Criminal Justice Services (DCJS) will no longer accept fax, mailed in or ITS requests from your offices or agencies for arrest information updates.

As of October 1, 2023, all requests for these documents must be submitted electronically to DCJS through the eJusticeNY Integrated Justice Portal (IJ Portal: www.ejustice.ny.gov).

After logging in to the IJ Portal, the new request form can be accessed as follows:

• Click Resources > Reference Library > Law Enforcement

• Once on the Law Enforcement section, click Forms and Publications (on the left side) and look for General Forms

• Arrest Supplement Form

No fax, mailed in or ITS requests will be accepted after October 1, 2023. If you do not have access to the IJ Portal or the Law Enforcement section, you must contact your Terminal Agency Coordinator (TAC) for access. If you do not know the name of your TAC, call DCJS State Identification Bureau at 518-457-6061. Thank you in advance for your cooperation as DCJS implements this new system.
